Community Team

SUFFOLK COMMUNITY HEALTHCARE - Hadleigh Local Healthcare Team

The Local Healthcare Team, which has two branches, is a multi-disciplinary team made up of District Nurses, Registered Nurses, Occupational Therapists, Physiotherapists, and support staff.

The team works with adults, aiming to help people maintain their independence, facilitate rehabilitation and meet their nursing care needs.

The Community Team is a quick response team, working seven days per week, including Bank Holidays. The aims of the team are to visit people in their home environment to provide interventions to prevent unnecessary admissions to hospital and allow people to be discharged from hospital as soon as possible. The nurses also manage individuals with leg ulcers in a clinic environment.

The Day and Treatment Team is a part-time service working on Mondays, Tuesdays and Thursdays. This team provides longer term interventions and rehabilitation programmes for people, in their own homes, or in the unit based at Hadleigh Health Centre.
