National Diabetes Audit (NDA)

This GP practice is taking part in an important national project about diabetes care and treatment in the NHS.  The project is called the National Diabetes Audit (NDA).

To take part, your GP practice will share information about your diabetes care and treatment with the NDA.  The type of information, and how it is shared, is controlled by law and enforced by strict rules of confidentiality and security. 

Taking part in the NDA shows that this GP practice is committed to improving care for people with diabetes.  

The NDA has always collected patient identifiable data from health care providers since it began in 2004.  However, people with diabetes need to be made aware that their service is participating in the NDA - this is known as 'fair processing'.  it does not mean that patients have to consent individually.

Operating from a purpose built Health Centre in Hadleigh and the Mill Surgery, our  branch surgery in Boxford; we provide a comprehensive service of family medicine to 15,000 patients.

All ten Doctors practice from Hadleigh with Drs Cope, Irwin, Glason, Croot, Everitt and Hill providing appointments at Boxford. Services are mainly based at the Hadleigh Health Centre where we have experienced Nurse Practitioners, Nurse Specialists, Practice Nurses and Health Care Assistants along with many visiting clinicians and out reach clinics.

The Practice is a member of the Ipswich and East Suffolk Clinical Commissioning Group www.ipswichandeastsuffolkccg.nhs.uk and we aim to work together with like minded Practices to provide all our patients and visitors a first class service at all times.  We would like to offer you a very warm welcome to our Practice.  Please explore our site for more detailed information on our range of services and how to contact us to access them.

GP practices are now required as part of the contract changes for 2015-16 to provide all their patients with a named GP who will have overall responsibility for the care and support their surgery provides to them.

As one of our patients, you have been allocated a named GP who will have overall responsibility for the care and support our surgery provides to you and will work with other healthcare professionals who are also involved in your care, to ensure that it meets your individual needs.

This does not prevent you from seeing any other clinician in the practice as you currently do now.
